Cuba, yes, and Toronto as well.

Americans can visit Cuba, the easiest way is to fly from Mexico or Canada.

I have met Americans in Cuba that were on the same flight from Toronto I was on! Guys from Western NY and Michigan come to Toronto for direct flights to Cuba.

Partially correct, yes. Cuban Americans are given permission to travel to Cuba (usually from Miami) every few years.

It is possible - with permission - for Americans with ties to Cuba (and journalists/politicians) to fly from the US to Cuba, otherwise, no.
