In operational security there's this concept called multi-signature, people at different locations having different portion of the key needed to access the cold storage
Hackers infiltrated the whole organization, targeted key people who had the access capability, issued a request to make a transfer to a hot wallet that the exchange owns for operational purpose, the security people verified everything, the destination address and the amount, the hack was so level computing that it involved the OS rending, meaning the security people were seeing an address and an amount in the software but the hackers were manipulating the rendering of what the security people were seeing in the user interface masking the real address and amount
These were elite hacking abilities, this becomes the biggest crypto hack ever
